One Health
Statens Serum Institut (SSI) arbejder for at styrke både menneskers og dyrs sundhed i det, der kaldes et One Health-perspektiv.
SSI varetager forsknings-, overvågnings- og infektionsberedskabsopgaver indenfor både det humane og det veterinære område. Dette betyder, at SSI officielt er et One Health institut.
Hvorfor er One Health relevant?
One Health tilgang er baseret på en forståelse for at der er sammenhæng mellem menneskers sundhed, dyrs sundhed og vores miljø. En One Health tilgang er vigtig i forbindelse med overvågning, diagnostik, udbrudsudredning og risikovurdering, da mange infektionssygdomme er zoonoser – de kan ramme både mennesker og dyr. Patogener med antimikrobiel resistens kan spredes imellem miljø, dyr og mennesker. Klimaforandringer, herunder forekomsten af ekstreme vejrfænomener, kan føre til øget udbredelse af flere smitsomme sygdomme, inklusive vektorbårne sygdomme. Miljøprøver, såsom spildevandsprøver, kan være relevante elementer i en integreret One Health overvågning.
For at varetage en omhyggelig overvågning af sygdomme og risikovurdering er det, kort fortalt, vigtigt at samarbejde tværfagligt.
Men One Health er også en vigtig tilgang, fordi der er mange ligheder mellem sygdomsprocesser i dyr og mennesker. Gennem tiden har forskere lært meget af hinanden ved at studere ligheder og forskelle mellem sygdomme i forskellige dyrearter og mennesker. Dette aspekt i One Health kan betegnes som komparativ forskning.
Endelig er der for et beredskabsinstitut som SSI en række fordele ved at anvende de samme teknologiske platforme og medarbejdere til at analysere prøver fra dyr og mennesker – alt sammen i et akkrediteret diagnostisk set-up. Samarbejdet giver dermed synergier, og derfor kan vi varetage et mere effektivt beredskab med en One Health-tilgang i modsætning til en opdeling af beredskabet i mere snævre discipliner.
Eksempler på SSI’s One Health aktiviteter
SSI har en lang tradition for at arbejde i et One Health-perspektiv, og SSI er involveret i både beredskabs- og overvågningsaktiviteter samt danske og internationale forskningsprojekter, som ud over det humane område også dækker dyr og miljø. SSI samarbejder i den forbindelse med en række samarbejdspartnere og netværk.
Udvalgte beredskabs- og overvågningsaktiviteter:
DANMAP
DANMAP samarbejdet overvåger antibiotikaforbruget til dyr og mennesker samt forekomsten af resistente bakterier fra enten dyr, fødevarer eller mennesker i Danmark. DANMAP samarbejdet blev etableret i 1995 og har årligt udgivet en rapport, der detaljeret beskriver både årets resultater fra overvågningen, men også udviklingen over de seneste 10 til 20 år. Derved har DANMAP bidraget væsentligt til at etablere og harmonisere de systemer, en sådan overvågning kræver, samt til etablering af vigtige nationale vejledninger omkring forebyggelse og håndtering af antibiotikaresistens. Samarbejdet nævnes ofte som et af de første One Health-initiativer i Danmark og internationalt.

KOZO
Sygdomme, der smitter både mennesker og dyr - zoonoser - kan ikke kun bekæmpes eller forebygges i én sektor. Vurderinger, ekspertviden og samarbejde på tværs sektorer er nødvendig. SSI deltager i KOZO, Myndighedsgruppen for KOordinering af ZOonoser, som varetager denne funktion i Danmark. KOZO mødes månedligt og efter behov og diskuterer aktuelle zoonotiske sygdomsudbrud og trusler fra både human- og dyre-sundhedsvinkler. Et fælles risikobillede dannes, hvorefter faglig kommunikation og respons kan koordineres.

Influenzavirusovervågning
På SSI overvåger vi influenzavirus i både dyr og mennesker. Dette indebærer både overvågning af cirkulerende virus i grise, fugle og mennesker, men også påvisning af alvorlige influenzatyper i henholdsvis dyr (fx fugleinfluenza af subtyperne H5 og H7) og mennesker (fx zoonotisk transmission).
Partnere: Overvågningen foregår i tæt samarbejde med nationale (SST, FVST) og internationale myndigheder (WHO, ECDC, WOAH).

Overvågning og smitteudredning af Ornitose (papegøjesyge) og aviær chlamydiose
Statens Serum Institut/Dansk Veterinær Konsortium overvåger i samarbejde med Styrelsen for Patientsikkerhed ornitose. Ornitose er en zoonose som kan overføres fra fugle inficeret med bakterien Chlamydia psittaci.

Tværfagligt samarbejde om fødevarebårne sygdomsudbrud
Statens Serum Institut indgår i et tæt samarbejde med Fødevarestyrelsen og DTU Fødevareinstituttet om koordination af fødevare- og vandbårne udbrud i Danmark. Samarbejdet er formaliseret i Den Centrale Udbrudsgruppe (DCUG) med faste repræsentanter fra de nævnte institutioner. Gruppen mødes ugentligt og overvåger løbende forekomsten af humane mave-tarminfektioner og sammenholder disse oplysninger med viden om forekomst af relevante agens i fødevarer og i husdyrproduktion.

SSI's VUA-BIO råder over en klasse III facilitet som kan bruges i OH sammenhæng, hvis der skulle opstå en beredskabssituation e.l. Faciliteten kan desuden bookes til løbende dyreforsøg.
VUAs pilot plant
SSIs VUA-PAV råder over en Up- og Downstream facilitet, som kan anvendes til fremstilling af vacciner til dyr i en One Health sammenhæng. Faciliteten råder også over en gennemgangsautoklave. Faciliteten kan desuden anvendes til fremstilling af humane vacciner til kliniske forsøg (dog foretages fyldning eksternt).

One Health AMR-highlight: DANMAP report 2024
The Danish Integrated Antimicrobial Resistance Monitoring and Research Programme (DANMAP) was established by the Danish Ministry of Food, Agriculture and Fisheries and the Danish Ministry of Health in 1995. The results are published each year in the DANMAP report, published by DTU National Food Institute and Statens Serum Institut.

The report is produced annually in a collaboration between the National Food Institute at the Technical University of Denmark, Statens Serum Institut, The Danish Veterinary and Food Administration and other collaborators.
